Funding of £668,299 has been made available by NHS Charities Together to support partnership work between community organisations and the NHS, working together to address health inequalities in South Yorkshire and Bassetlaw exacerbated by the Covid-19 pandemic.
• Preventing avoidable admissions to NHS facilities,
• Supporting discharge into the community,
• Supporting mental health and wellbeing.
Grants are specifically to support partnership working between community organisations and the NHS. Applicants should be community organisations already working with the NHS, not speculative proposals.
Grants of £83,500 minimum (and no maximum value) are available for up to 24 months.
Note: the total pot of funding for the region is £668,299.
It is a requirement of this funding that your grant should be spent within 24 months of the award. Please keep this in mind when deciding how much your organisation applies for.
